| Description: | Provide hands-on supervision of caregiver staff during their shift, ensure provision of quality resident care, and uphold community philosophy and resident well being. Provide direct assistance with activities of daily living and support to residents. |
| Duties: | Supervise the caregiver staff on duty in providing quality care and meeting the needs of residents in accordance with community philosophy and policies.
Observe for significant change in resident's condition, report to resident's physician, family member or designee and document immediately.
Ensure that assigned caregivers utilize resident care plans in delivery of nursing care.
Assist residents with their activities of daily living; check on residents routinely and document and report findings.
Assist with resident activities.
Assist with serving meals and supervise caregiver staff in the dining room; ensure that dining room is cleaned after each meal service.
Assist with cleaning and tidying residents' rooms/apartments.
Establish and maintain good relationship with residents and their families.
Ensure that all systems for providing and tracking resident care are completed in accordance with the community's policies and procedures.
Participate in the development of Resident Care Plans; assist with resident admissions and discharges, modify Resident Care Plans to reflect current health status of residents.
Supervise the delivery of nursing care on assigned shift and assigned unit(s); ensure that all prescribed medications and treatments are administered and documented; ensure nursing care is delivered with respect for residents' rights.
Provide direct resident care; assist Caregivers with difficult assignments; responsible for documentation and charting; ensure proper Medicare documentation, as required.
Ensure the proper and timely response to all resident requests (verbally or call-light) for assistance in accordance with community guidelines.
Ensure adequate staffing of Caregivers to maintain continuity of resident care; inform supervisor or the Director of Health Services of all scheduling and staffing problems.
Practice positive resident relations and respond timely and appropriately to all family inquires or requests in accordance with company guidelines.
The facility operates 7 days a week, 24 hours a day. Department schedules must accommodate fluctuating business demands and employees may be asked to work shifts other than those they prefer or normally work.

| Qualifications: | Registered Nurse (RN) or Licensed Practical Nurse (LPN) state license is required (or must have applied for transfer of license from another state).
One (1) to two (2) years experience, preferably in long-term care or a related field.
Above average knowledge of professional medical practice and care.
Above average knowledge of organization policies, regulations, and procedures to administer resident care.
Above average skill in taking medical history and vital signs, assessing medical conditions, and interpreting findings.
Above average ability to exercise initiative, judgment, and decision-making in meeting clinical objectives.
Above average knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to extended-care facility operations.
Above average ability to interact and communicate with a variety of people, both on a one-on-one basis and in meetings and group presentations; must be able to relate to and work with ill, disabled, emotionally upset, and sometimes hostile people; must be able to communicate in a manner understandable by older adults.
Above average ability to think logically in order to troubleshoot, analyze situations, and make sound decisions.
Average ability to make independent decisions.
Must be flexible and willing to adjust to changing environments and schedules.
Average ability to write and edit reports and correspondence.
Working knowledge of mathematical principles to make calculations.
Basic understanding of computer applications, including Microsoft Office, e-mail systems, and Internet browsers.
Ability to handle multiple tasks simultaneously.
Ability to assume on-call responsibility occasionally.
